Your first visit to Low Country Dermatology involves a few special steps so that we can get to know you. To understand what to expect, please read through this page.
Prior to your initial visit, you will be asked to fill out patient registration forms to help us get to know you, including your medical and dermatologic history. These forms can be printed out and brought to the office on your first visit, or they can be completed online. We also ask that you look over our financial and privacy policies, and return the signature portion of that form at your fist visit. Please make sure to provide your email address when you are setting up an appointment with us or in your new patient paperwork so we can get your Patient Portal set up.
